Overview
As a member of the interdisciplinary team, contributes professional nursing knowledge and skills in the provision and management of care to patients through the application of the nursing process.
Qualifications- License/
Certifications:
Basic Life Support (BLS) within 30 days - American Heart Association (AHA);
Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR) - Education:
College Diploma in Nursing;
Nursing Diploma
- Promotes the use of evidence-based practice through nursing protocols and procedures.
- Assesses patient status via interview, observation, and physical assessment of problems and outcomes.
- Develops an individualized plan of care collaboratively with patient/family and health‑care team.
- Implements interventions identified in the care plan to achieve expected patient outcomes.
- Evaluates and documents patient progress toward outcomes on an ongoing basis throughout the shift.
- Modifies the care plan as needed based on evaluation and patient condition changes.
- Completes required documentation, including patient chart and event reports.
- Demonstrates technical knowledge of equipment and troubleshoots problems when they arise.
- Shares clinical knowledge through informal teaching and in response to others’ learning needs.
- Performs bedside lab testing and quality control in compliance with provider orders and hospital policies.
- Coordinates interdisciplinary care through collaboration with team members and additional resources when necessary.
- Analyzes clinical situations systematically and obtains resources or corrects problems to meet or exceed outcomes or patient needs.
- Recognizes when assistance is needed and utilizes resources, including physician and nurse‑to‑nurse consultation, in a timely manner.
- Prioritizes work based on patient condition changes, assignments, and interruptions.
- Identifies and utilizes team resources to solve problems or meet needs.
- Organizes tasks and activities from admission to discharge according to the care plan to meet expected outcomes.
- Provides goal‑focused shift reports.
- Advocates with physicians on behalf of patients when necessary.
- Uses communication and teamwork strategies to promote high team performance and optimal patient outcomes and unit productivity.
- Selects teaching strategies appropriate to patient/family age and needs and adapts them based on response, readiness, and comprehension.
- Ensures patient safety by following policies, procedures, and National Patient Safety Goals.
- Consults appropriate hospital resources to provide specialized education.
- Utilizes the SVI patient education system for patients and families.
- Participates in data collection, analysis, and problem solving, contributing to process improvements and patient satisfaction.
- Ensures care aligns with standards of care, evidence‑based practices, national safety goals, core measures, and quality initiatives.
- Collaborates with interdisciplinary teams to identify opportunities for patient and family satisfaction improvements.
- Participates in the implementation of performance improvement initiatives.
- Promptly and efficiently follows up on patient/family requests and anticipates their needs.
- Serves on unit‑based councils.
The compensation range for this position is $36.8 per hour – $59.89 per hour. This represents a good‑faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered to a candidate will depend on experience, qualifications, location, training, licenses, shifts worked, and compensation model.
