Overview
We are hiring a Hospice Admission RN to join the admission team in Belleville, IL. The role provides compassionate, end‑of‑life care in home‑like settings and includes assessment, care planning, and coordination with physicians, families, and the interdisciplinary team.
Position Summary- Full‑time, non‑exempt; 40‑hour week with hourly rate $39.36.
- Schedule:
Monday‑Friday 8:30 AM‑5:00 PM; optional 12:00 PM‑8:30 PM. Rotational on‑call covering evenings, weekends, and holidays. - Location:
Belleville, IL office; travel within St. Clair, Monroe, Randolph, Washington, Madison, Clinton, Bond, Montgomery, Macoupin, Jersey, and Calhoun counties. Valid driver’s license, reliable transportation, and mileage reimbursement of $0.70 per mile required. - Typical workflow: 1–2 admissions per day with completion of referral, physician certification, and documentation.
- Coordinate intake, collect information, complete referral forms, and document the pre‑admission process.
- Educate patients, families, and referral sources on hospice philosophy, services, and admission consents.
- Conduct comprehensive physical and psychosocial assessments; document findings and collaborate with physicians to determine eligibility and care plans.
- Use Medicare Hospice Eligibility Guidelines and other regulations to evaluate eligibility.
- Explain medication, durable medical equipment, and care needs to families.
- Engage in interdisciplinary team communication to ensure timely start of care.
- Assist with routine patient visits, on‑call shifts, and care management as needed.
- Participate in QAPI activities, performance improvement, and referral process improvements.
- Maintain accurate documentation and timely timesheet submissions.

- Education
:
Associate Degree in Nursing or Nursing Diploma from an accredited program. - Licensure
:
Current Illinois RN license. - Experience
:
Minimum one year in hospice preferred. - Preferred Certifications
:
Hospice and Palliative Care, CPR. - Skills
:
Clinical assessment, leadership, organization, interpersonal communication; knowledge of Medicare/Medicaid and hospice regulations. - Transportation
:
Valid driver’s license, reliable vehicle, insurance, and willingness to drive within service territory. - Physical Capacity
:
Able to lift 20‑100 lbs., bend, squat, and work in varied environments (home or office). - Other
:
Ability to manage multiple priorities, work under pressure, and maintain professionalism.
- Repetitive tasks, lifting up to 100 lbs. occasionally, 50 lbs. frequently, 20 lbs. continuously; bending, squatting, kneeling.
- Work 50% in a controlled environment; 50% in patient homes or vehicles with variable conditions.
- Exposure to body fluids, odors, and infectious risks; ability to hear alarms, use a phone/tape recorder.
- Requires visual acuity of at least 20/20.
- Time spent in air‑conditioned office or external client sites.
- Exposure to noise, temperature variation, allergies, and potential trauma.
- Travel required across many counties.
- Hourly rate $39.36.
