OSF Oncall Mobile Community Care
OSF Mobile Community Care is hitting the road with a new Primary Care Mobile Vehicle. We are seeking a dynamic, self‑motivated, independent provider to reach patients where they are, utilizing in‑person and virtual care to support our rural communities. This experienced provider will engage patients from various populations, especially those underserved or with limited access to healthcare, across Central, East, and Northcentral Illinois.
While the focus is adult care, familiarity with pediatric and infant care is encouraged.
- Unique opportunity to provide in‑person and virtual care to rural underserved communities.
- Serve as the Primary Care Provider.
- Collaborate with leadership to enhance community engagement.
- Deliver high‑quality, safe patient care.
- Collaborate with nursing and provide support to enable 24/7 access to care for this patient population.
- Support a culture of innovation and growth, encouraging open discussion of ethical and clinical concerns and real‑time feedback.
- Coverage includes Fulton, Henry, Bureau, Winnebago, LeSalle counties and more.
- Schedule:
40 hours a week (travel to locations included in weekly requirements). - Epic EMR required.

- Clinical base hourly pay range: $55.20–63.65hr; salaried position.
- Potential bonuses or incentives may apply.
- Education:
Master’s degree in advanced practice nursing. - Experience:
Demonstrated knowledge and skills for appropriate patient care across ages served. - Licensure/Certification: IL Advanced Prac Nurse, IL Reg Nurse, Fed Controlled Sub‑Lic, BLS/ACLS Certified.
- Excellent interpersonal and communication skills.
- Proficiency with Microsoft software and other computer applications.
- Strong analytical and problem‑solving skills, with attention to detail.
- Experience with EPIC electronic health record.
- 1–2 years of experience as an Advanced Practice Nurse.
