Job Summary
Gentiva is seeking a Registered Nurse (RN) Hospice to provide nursing care to patients with life‑limiting illnesses in Peoria, Illinois. The role supports patients and families, delivering personalized, compassionate care and working closely with interdisciplinary teams.
Responsibilities- Serve as the primary point of contact for patients and families, guiding them through the hospice journey in the patient’s home or other care settings.
- Conduct initial and ongoing assessments, document skilled nursing interventions, and update individualized plans of care.
- Collaborate with physicians, LPNs, CNAs, social workers, chaplains, and other disciplines to refine care plans and coordinate services.
- Provide emotional, physical, and spiritual support to patients, families, and caregivers, offering education and bereavement guidance.
- Teach caregivers safe care practices and offer emotional reassurance.
- Communicate patient condition changes to the care team and ensure timely interventions.
- Maintain accurate clinical documentation and adhere to infection control and compliance standards.
- Support quality improvement, scheduling initiatives, and continuity of care across care transitions.
- Participate in on‑call and patient support activities as required.

- Education:
Registered Nurse (RN) license. - Experience:
Minimum 1 year as an RN in acute care hospital, oncology, long‑term care, hospice, or home health; hospice or palliative care experience preferred. - Licenses/Certifications:
Current state nursing license, CPR certification, valid driver’s license. - Knowledge:
Experience with terminally ill patients, hospice care principles, regulatory guidelines (Medicare, Medicaid, ACHC), and community resources. - Skills:
Excellent patient assessment, documentation, communication, and program‑management abilities. - Traits:
Sensitivity to life‑end issues, flexibility, effective communication across socioeconomic backgrounds, and commitment to teamwork.
- Salary Range: $71,450 – $89,335
