LPN – Home Health

• Assist in providing high-quality nursing care to Home Health Services patients under the direction and supervision of an RN
• Follow the RN-established plan of care and conduct client assessments as appropriate
• Complete visit documentation and clinical records according to Carle Health policies, procedures, and regulatory requirements
• Implement nursing interventions based on client needs and changing conditions
• Communicate changes in client condition and client/caregiver needs to RNs and other care team members
• Participate in client case reports and coordinate care with Carle Health staff, physicians, and other resources
• Perform observations, test procedures, lab testing, and measures to prevent complications or minimize disease or injury effects
• Provide intermittent skilled care in compliance with the Plan of Treatment
• Maintain complete, legible, accurate, and timely clinical records, including daily visit notes, change orders, audit issues, occurrence reports, and follow-up communications
• Organize visit schedules, prioritize caseload needs, and maximize client care
• Attend quarterly departmental meetings and participate in departmental work groups/teams
• Educate clients, caregivers, and home health team members about health information
• Report to the assigned Home Health Clinical Manager

• Licensed Practical Nurse (LPN) certification from the Illinois Department of Financial and Professional Regulation (IDFPR)
• Illinois driver's license from the Illinois Secretary of State (ILSOS)
• Proof of auto insurance
• Basic Life Support (BLS) certification from the American Heart Association within 30 days
• 1 year of nursing experience
• Must meet and maintain departmental and Carle Health educational requirements and required certifications
• Must obtain and maintain Carle Health technical skills, competencies, or certifications required for Carle Health and specific client populations
• Knowledge of payor source requirements, Standards of Professional Practice, Carle Health policies and procedures, and regulatory requirements
• Ability to provide care appropriate to clients of all ages and groups
• Ability to work scheduled home health visits and cover extra skilled nursing visits or weekend/holiday p.m. visits when requested

Demonstrates expertise in providing high-quality nursing care in home health settings, adhering to established plans of care and regulatory requirements. Proficient in client assessments, documentation, and effective communication with care teams to ensure optimal patient outcomes.
