American Traveler is hiring a RN Case Manager for an acute Continuing Care Hospital, requiring an active IL RN license and 2 years of case management/discharge planning experience.
Details- Acute care setting within a Case Management/Utilization Review department at a Continuing Care Hospital
- Covers Inpatient Physical Rehab and Long Term Acute Care (LTAC) populations
- Patient population includes complex LTAC cases (vents, airways, IV antibiotics, respiratory, wounds, chronic/acute illness) and Rehab cases (neuro, amputation, cardiac, trauma, medical/surgical complex)
- Caseload of approximately 13-25 patients per day
- Uses EPIC EMR
- MCG criteria used for utilization review, training provided if needed
- Schedule is Monday through Friday, 8:00am-4:30pm, day shift, 8-hour shifts
- No holiday coverage required
- Occasional flexible call availability for questions
- Team conferences held Monday through Friday for Rehab, with interdisciplinary rounding on LTAC (ICU Mondays, Med Surg Wednesdays)
- Active IL RN license required (Social Worker or Clinical Social Worker license also accepted)
- BLS certification (AHA) required
- Minimum 2 years of experience required
- Healthcare setting experience preferred, including discharge planning and care coordination in an acute hospital setting
- Background in LTAC or Rehab preferred but not required
- Strong computer skills including Microsoft Office proficiency
- Strong analytical and problem-solving skills
- Excellent interpersonal and verbal/written communication skills
- Ability to interact with diverse populations
- Ability to multitask and prioritize in a fast-paced environment
- Basic understanding of acute and post-acute levels of care and resources
- Responsible for developing and managing individualized care plans for assigned patients from admission through discharge
- Collaborates with patients, families, and multidisciplinary teams to support patient progression through the continuum of care
- Manages length of stay and resource utilization in an efficient, cost-effective manner
- Duties include interdisciplinary rounding, utilization review, insurance authorization, concurrent reviews, discharge planning, DME coordination, and post-discharge referrals
- Coordinates home health vs. outpatient referrals, transportation, and other discharge resources
- Assists with setting up ventilator facilities and coordinating pharmacy needs (including anticoagulants, IV antibiotics, and dialysis referrals) for discharge planning
- Facilitates family meetings and discussions regarding advanced directives and surrogacy
- Orientation includes facility and departmental training along with regulatory requirement review, with additional training provided for candidates without LTAC or Rehab experience
- Dress code is scrubs or business casual, no colors
- Local rate applies for clinicians residing within 50 miles of the facility
- No radius restriction for eligibility
- Negative drug screen required for all panels
