Aging Life Care Specialist (Geriatric Care Manager)

We are hiring an Aging Life Care Specialist at Aging Life Care Association®. The position is full-time, 40 hours a week with on‑call responsibilities one week per month or part‑time hours. The role offers the opportunity to advance to a supervisory position for the right individual. The compensation is hourly plus mileage, and health insurance is available after the probationary period with the employer compensating part of the premium.

Responsibilities:

• Client visits and assessments regarding resources and levels of care.
• Developing and monitoring care plans.
• Coordinating physician appointments.
• Advocating for clients and staying familiar with local senior resources.
• Managing own caseload as a self‑starter who enjoys advocacy.

Qualifications:

• MSW, BSW, CMC, RN, LPN, or LF degree or certification.
• Clinical experience with elderly population and mental health preferred.
• Background check, clean driving record, reliable transportation and insurance.
