Full-Time (Days, Monday through Friday, 8-Hour Day Shifts)

• Provide consistent nursing care for patients with chronic conditions
• Administer medications and treatments according to provider orders
• Monitor patient conditions and document responses
• Assist with care plans, assessments, and interventions
• Collaborate with RNs and providers to ensure quality outcomes
• Follow facility protocols and correctional healthcare guidelines

• Active LPN license in Georgia
• Experience or interest in chronic care management
• Strong clinical skills and attention to detail
• Ability to work independently and as part of a team
• Comfortable in a correctional healthcare environment
• Commitment to compassionate, patient-centered care
