Gentiva Hospice is seeking a compassionate Hospice RN / RN Case Manager to lead patient care in home settings. You will perform assessments, deliver skilled nursing interventions, and coordinate with physicians, social workers, and other team members to implement personalized care plans.
Must have active RN license and hospice experience. You will educate patients and families, document care activities accurately, and participate in on-call coverage as needed.
