Home Health RN Case Manager & Recertification Nurse (RNCM)

Position Overview: A Part‑time or PRN “Road Warrior” leadership position requiring travel throughout Northwest Ohio, reporting to the regional program administrator.

An RN Recertification Nurse will coordinate, re‑evaluate, and confirm the medical necessity of continued home health care services, providing patient care and ensuring optimal outcomes. Responsibilities include managed care, patient assessments, compliance, documentation, discharge planning, and communication with physicians, staff, and clients.

• Perform client assessments and initiate appropriate actions in home care settings.
• Manage and supervise a team of RN, LPN, Home Health Aide, and Personal Care Aides.
• Document, communicate, and coordinate patient status changes with the care team.
• Maintain safe environment and personal safety for clients and coworkers.
• Travel to surrounding counties as required.
• Provide occasional skilled nursing visits and services such as med sets and dressing changes.

• Graduate of an accredited school of nursing and active RN license in Ohio.
• Minimum of 1 year experience as a registered nurse with an active RN license.
• Minimum 1 year experience in a home health agency or equivalent setting.
• Experience with OASIS and non‑OASIS assessments.
• CPR and First Aid certification (or ability to obtain before hire).
• Strong background in nursing, patient care, confidentiality, and HIPAA regulations.
• Exceptional on‑time attendance and consistent availability aligned to chosen schedule.
• Willingness and ability to perform required physical demands and travel requirements (up to 2‑hour travel radius).
• Valid driver’s license and reliable transportation.
• State and federal background check clearance.
• Desirable: experience supervising nurses, people management, education/development, and building relationships with payors and stakeholders.

Road Warrior position covering the entire Northwest Ohio region (Toledo, Holland, Perrysburg, Fremont, Norwalk, Defiance, Findlay, Richland, Marion, Mansfield, Ashland, Fostoria, Tiffin, and surrounding areas). Requires willingness to travel within the region. Daytime availability Monday‑Friday (8 am–5 pm) with occasional evenings; no overnight travel or on‑call requirements. No weekend shifts required.

Part‑time or PRN hourly rate: $36–$40 per hour plus mileage reimbursement. Paid weekly via direct deposit.
