Home Health RN Case Manager & Recertification Nurse (RNCM)
Job OverviewPart‑time or PRN "Road Warrior" leadership position requiring travel throughout Northwest Ohio. Reports to the NW OH Regional Program Administrator and supports a defined career path to higher roles.
Responsibilities- Perform client assessments, case management, admissions, recertifications, transitional care, resumptions of care, and clinical reviews.
- Document and communicate patient status changes to the care team.
- Supervise and coordinate RN, LPN, HHA, and PCA staff.
- Provide skilled nursing services (med sets, dressing changes) as needed.
- Contribute to training, education, and quality improvement initiatives.
- Travel to client sites; no overnight travel or on‑call shifts.
- Graduate of an accredited School of Nursing with an active RN license in Ohio.
- Minimum 1 year experience in home‑health or an equivalent setting.
- At least 1 year experience in case management, admissions, wound care, PICC/PORT/IV care, transitional care, lab draws, OASIS/Non‑OASIS assessments, and documentation.
- CPR and First Aid certifications (obtainable before hire).
- Experience supervising RN, LPN, HHA, or PCA staff.
- Strong communication, HIPAA knowledge, and physical ability to perform required tasks.
- Background check and fingerprints required.
- Valid driver’s license, reliable transportation, and ability to travel up to 2 hours.
Weekday (Monday–Friday) daytime availability 8 am–5 pm with occasional evenings. No weekend or overnight shifts. Service area includes all counties in Northwest Ohio:
Toledo, Holland, Perrysburg, Fremont, Norwalk, Defiance, Findlay, Richland, Marion, Mansfield, Ashland, Fostoria, Tiffin, and surrounding areas.
& Compensation
Long‑term employment, Part‑time or PRN RNCM/Recertification Nurse. Hourly pay $36–$40 per hour, plus mileage reimbursement, overtime, and quarterly bonus opportunities. Pay is released weekly via direct deposit.
