Home Health RN Case Manager & Recertification Nurse (RNCM)
OverviewThis Part‑Time or PRN “Road Warrior” role requires travel throughout Northwest Ohio to provide case management, admissions, recertifications, transitional care, and clinical reviews. The RNCM serves as a field staff leader, overseeing a team of RNs, LPNs, HHAs, and PCCAs, and plays a key role in quality compliance, training, and client outcomes.
Schedule & Travel- Weekday, Monday – Friday; primarily 8 a.m. – 5 p.m.; occasional evening service possible.
- No overnight travel or night shifts.
- Comprehensive coverage of the entire North West Ohio region, including Toledo, Holland, Perrysburg, Fremont, Norwalk, Defiance, Findlay, Richland, Marion, Mansfield, Ashland, Fostoria, Tiffin, and surrounding areas.
- Road‑warrior position; travel required as needed to all client sites.
- Part‑time or PRN: $36 – $40 per hour + mileage reimbursement.
- Paid weekly on Fridays via direct deposit.
- Overtime and quarterly bonus opportunities available.

- Conduct client assessments, admissions, and discharge planning.
- Perform recertification and
** clinical reviews
** to confirm medical necessity. - Document and communicate patient status to the care team.
- Supervise and coordinate staff (RNs, LPNs, HHAs, PCCAs).
- Ensure compliance with state, federal, and agency policies.
- Provide back‑up skilled nursing services (med sets, dressing changes, etc.).
- Participate in new‑hire orientation, in‑service trainings, and supervisory reviews.
- Support continuous quality improvement and training initiatives.
- Minimum 1 year experience in case management, admissions & discharges, wound care, PICC/PORT/IV care, transitional care, lab draws, and OASIS/Non‑OASIS assessments.
- Minimum 1 year paid experience in a home health or in‑home care setting; 2–5 years preferred.
- Active RN license in Ohio.
- CPR and First‑Aid certifications (required).
- Familiarity with HIPAA regulations.
- Physical ability to perform required duties.
- Background check and fingerprints required.
- 18 years of age or older, active driver’s license/State , and U.S. work authorization.
- Reliable transportation capable of travel within a 2‑hour radius of scheduled clients.
- Strong candidate traits: reliable, professional, compassionate, detail‑oriented, accountable, and collaborative.
- Nurse supervision experience.
- People management and peer/staff education.
- Relationship building with payors and case‑management entities (e.g., AAA, DoDD, County Boards, VA).
