- Provide direct patient care to residents, including assistance with activities of daily living (ADLs), medication administration, and vital sign monitoring
- Develop and maintain positive relationships with residents, families, and interdisciplinary team members
- Participate in resident assessment, care planning, and documentation
- Collaborate with other healthcare professionals to ensure seamless coordination of care
- Maintain accurate and timely records of patient care
- Perform restorative care activities, such as ambulation, range of motion exercises, and toileting assistance
- Adhere to Spring Creek Healthcare Center's policies and procedures, as well as applicable state and federal regulations
- Current CNA certification in the state of New Jersey
- High school diploma or equivalent required
