• Provide individual and group support to residents and their families.
• Conduct psychosocial assessments and develop care plans.
• Collaborate with interdisciplinary teams to ensure holistic care.
• Maintain accurate and timely electronic records.
• Advocate for residents’ rights and access to community resources.

• Active Licensed Clinical Social Worker (LCSW) New Jersey.
• Compassion and a genuine interest in working with the senior population.

• Medicare/Medicaid credentialing is a plus or willingness to obtain credentials.

• Ready to hit the ground running and make a difference!
• Willing to work onsite

This role requires you to work onsite 100% of the time.
