Responsibilities- Collect comprehensive data pertinent to the patient's health.
- Perform age- and disease-specific assessment and data collection independently.
- Recognize clinical and diagnostic status changes in patients.
- Identify variables in patient/family education needs based on age, disease, and culturally specific learning differences.
- Develop a plan of care that prescribes strategies and interventions to attain expected outcomes.
- Formulate age- and disease-specific patient/family centered goals/outcomes based on assessment data.
- Develop and coordinate a plan of care that is patient/family focused, prioritized, and encompasses the continuum of care.
- Communicate, coordinate, and implement the plan of care.
- Provide ongoing communication with patient/family, nursing colleagues, and other members of the health care team regarding patients' status, rendered care, and treatment plans.
- Implement age- and disease-specific care in an organized and timely manner.
- Provide or coordinate patient/family education as needed and ensure understanding.
- Perform according to established protocols in clinical situations, including communicating with appropriate persons and documenting events.
- Provide nursing care and treatments that reflect the patients' needs, advocate appropriately, and demonstrate evidence-based nursing practice and compliance with standards.
- Perform continuous point-of-care electronic documentation of each patient's assessments, plan-of-care, interventions, and evaluation of rendered care.
- Administer medication utilizing bar-scanning and documentation technologies to ensure the safest delivery of medications.
- Engage in multidisciplinary discharge planning to ensure safe transitioning and continuum of care.
- Evaluate progress toward attainment of outcomes.
- Utilize critical thinking skills to achieve desired patient outcomes.
- Initiate interventions related to patients' change in condition and unexpected response to care.
- Identify ethical issues in the practice setting and bring them to the attention of other team members.
- Recognize and respond to opportunities to enhance patient satisfaction.
- Maintain required competencies, certifications, and continuing education to meet facility accreditation and licensing requirements.
- Adhere to the American Nurses Association Code for Nurses with Interpretative Statements.
- Meet physical demands: lift a minimum of 35 lbs., push and pull a minimum of 10 lbs., and stand for a minimum of 6 hours a day.
- Adhere to HMH's Organizational and Managerial competencies and standards of behavior.
- BSN in progress and/or willing to acquire within three (3) years of hire (applicable to hires on or after November 2019).
- Six (6) or more months of relevant nursing experience or active participation in Hackensack Meridian Health's Nurse Residency Program.
- Strong ability to learn quickly and adapt to changing patient needs, a sense of accountability for improving patient lives, a focus on teamwork, dedication to ongoing education, and passion for delivering high-quality care.
- National certification in area of specialty when eligible.
- NJ State Professional Registered Nurse License.
- AHA Basic Health Care Life Support HCP Certification within 60 days of entering position.
Minimum rate of $45.73 hourly. Compensation is benchmarked against market data and may vary based on experience, education, certifications, geographic location, and other factors. Additional incentives, shift differentials, and benefits may apply.
