Nurse Practitioner​/Physician Assistant

We are hiring a Physical Medicine & Rehabilitation (PM&R) / Physiatry NP/PA clinician to provide specialty services in a skilled nursing facility or post‑acute rehabilitation center in Perth Amboy, NJ.

• Provide specialized PM&R/Physiatry services as a rehab consultant
• Represent the practice through clinical leadership and collaboration with the interdisciplinary care team
• Collaborate with PCP teams to manage common SNF conditions such as cerebrovascular accident, progressive neurological disorders, and postoperative orthopedic surgeries
• Work alongside PT, OT, and SLP teams to improve patient function and quality of life
• Follow therapy patients to track their progress in therapy
• Assess and collaborate on the management of barriers to therapy including joint or neuropathic pain, spasticity, neurogenic bowel, neurogenic bladder, and more
• Attend multidisciplinary meetings to bridge the gap between therapy and internal medicine
• Engage in culture‑building opportunities and ongoing education, including monthly didactic lectures, virtual meetings with the regional team, and annual regional dinners

• A passion for post‑acute healthcare and serving the geriatric population
• Experience in SNF or post‑acute settings (preferred)
• Confidence to function autonomously

• Nurse Practitioner/APRN/PA license
• New Jersey license
• U.S. work eligibility
• Experience in the post‑acute space preferred

Location:

Perth Amboy, NJ

Role: 32‑hour W‑2, full‑time

Schedule:

4‑day work week, eight‑hour shifts, 9 AM–4 PM (flexible days Monday–Friday)

Pay: $95,000 – $125,000, plus bonus and other incentives

Required:

Reliable commute or relocation to Perth Amboy, NJ before start of employment.
