Case Manager, Early Intervention Program
Provides clinical care coordination in conjunction with healthcare providers and other support staff. Promotes prevention and health maintenance in an ambulatory care environment. Performs psycho-social assessment of the EIP patients and their families to determine and assist in resolving social, emotional and economic problems.
Age/Patient Population served:
Adolescent (13–17 years), Adult (18–64 years), Geriartric (65+ years).
Skills and Abilities
Required
- Bachelor's degree in related field and/or four years clinical experience with individuals with HIV infection in either the acute care or ambulatory care setting
- Must be able to manage difficult or emotional customer situations; should be able to focus on solving conflicts and problem resolution
