• Evaluate and treat patients in a skilled nursing facility setting
• Develop and implement individualized physical therapy treatment plans
• Document patient progress and response to treatment accurately and in a timely manner
• Collaborate with interdisciplinary healthcare team to deliver comprehensive care
• Educate patients and caregivers on exercises, safety, and home programs
• Maintain compliance with facility standards and state/federal regulations

• Active PT license in IL required to start the assignment
• Doctorate of Physical Therapy (DPT) from an accredited program
• BLS/CPR certification
• Prior SNF experience preferred
• Mentorship program with Board-Certified Specialists
• PT-led recruitment: your recruiter is a licensed, actively practicing physical therapist
• Housing stipend provided
• Double-sided referral bonus: $600 per referral ($1,200 total) when both you and your referral complete a 13-week contract
• CEU reimbursement: $400 per contract, up to $1,600/year
• Full licensure reimbursement: 100% of new and transfer state license costs covered
• Relocation stipend up to $600 per assignment
