Registered Nurse

Job Summary:

The Registered Nurse provides skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. This role involves patient assessments, medication administration, treatment planning, and collaboration with an interdisciplinary team. Strong communication and collaboration skills are essential for working with patients, providers, and team members.

Responsibilities:

- Conduct patient assessments to determine care needs.
- Administer medications as prescribed.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team to coordinate patient care.
- Provide skilled nursing care to patients.
- Manage patient cases to ensure optimal outcomes.
- Provide hospice care to patients as needed.
- Maintain accurate and up-to-date patient records.

Required Skills &

Certifications:

- Active Registered Nurse license in the state of working.
- Bachelor's degree in Nursing or related field.
- Clinical Assessment skills.
- Medication Administration skills.
- Treatment Planning skills.
- Interdisciplinary Team Collaboration skills.
- Minimum of 2 years of hospital, home health, or hospice care experience.
- Driver's license, car insurance, and automobile for field work (if applicable).

Preferred Skills &

Certifications:

- Specialities in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room.
