• Utilize clinical skills to create, implement, and execute the overall plan of care to promote wellness and prevent rehospitalization.
• Assess patients and families at the initial visit and develop advanced plans of care including rehabilitation.
• Provide teaching, oversight, and assistance for wound care, blood draws, tube feeding, tracheostomy care, Foley catheters, CHF management, and medication education.
• Case‑manage patients’ care episodes with an interdisciplinary clinical team.
• Prepare and chart clinical notes, orders, and documentation within required time frames.
• Collaborate with physicians and agency staff to deliver optimal care.
• Initiate and administer IV medications (antibiotics, diuretics) following established protocols.

• Wage: $130,000 - $160,000 plus $10,000 annual bonus, paid quarterly.

• Current and unrestricted California RN license.
• Current BLS/CPR certification.
• Valid California driver’s license.
• High level of clinical knowledge in home health care preferred.
• Experience with high‑acuity patients is a plus.
• Experience working with interdisciplinary teams.
• Fundamentals of case management.
• Previous experience with IV medication infusions preferred.
• Proficiency initiating intravenous therapy.
• Self‑directed, empathetic, critical thinker.
• Outstanding interpersonal and leadership skills.
• Organized.
