The Case Manager is responsible for coordinating the delivery of Nursing and Rehabilitation services to our residents in collaboration with the facility’s care staff. Takes ownership of all admissions as they enter into the facility, communicating with the resident/RP. Works very closely with the IDT and Director of Nurses. The case manager is to serve as patient advocate and liaison to insurance carriers and other medical management professionals, and is in charge of managing the nursing portion of our resident care conferences.
Works closely with IDT and Social Services to assist in planning and executing a safe discharge.
- Have an active LVN or RN license
- Prior experience in skilled nursing preferred but not required (if not, have you work in a field related to case management?)
- Outstanding communication (oral and written) skills
- Ability to multi-task and treat everyone at work kindly and professionally
- Works collaboratively and independently, following procedures and guidelines, making recommendations for improvements
- Completes tasks on time, reporting up on successes, challenges, and offering solutions to problems
Emotional maturity – ability to remain calm and professional under stress
- Greet new patients/residents ensuing a positive admission experience.
- Evaluate and communicate resident status, changes in functions and care plans to payers
- Document all payer interactions regarding resident progress, expected outcomes and special instructions
- Meet with interdisciplinary team to coordinate services ensuring resident’s total regimen of care is maintained
- Coordinate and communicate with business office and medical records to ensure accurate census and payment of Managed care and Medicare residents
