Pay: $23 – $25 per hour (DOE)
Full time schedule / On-site Position Ridgway Post Acute is seeking a caring and organized Social Services Assistant to support our residents and families. This is a great opportunity for someone who enjoys helping others and wants to grow in the healthcare field while making a real difference in residents’ lives.
You’ll work closely with our Social Services Director and interdisciplinary team to help residents access resources, coordinate services, and maintain the highest level of well-being.
What You’ll DoAssist the Social Services Director with social service programs and resident support
Help coordinate appointments, services, and community resources
Support resident and family communication
Assist with discharge planning and care coordination
Maintain social service documentation and reports
What We’re Looking For Associate’s or Bachelor’s Degree preferred (not required)
Healthcare experience preferred
Strong communication and organizational skills
