Are you a compassionate and skilled Nurse Practitioner with a passion for cardiological care? Monadnock Community Hospital (MCH) is seeking a Cardiology Nurse Practitioner to join our dedicated team of healthcare professionals committed to delivering exceptional patient-centered care.

This is an exciting opportunity to work collaboratively with cardiologists, physicians, and multidisciplinary healthcare teams while helping patients manage acute and chronic cardiological conditions. What You'll Do

As a Cardiology Nurse Practitioner, you will:

  • Perform comprehensive cardiological assessments, including detailed patient histories and physical examinations.
  • Evaluate, diagnose, and manage patients with a wide range of cardiac conditions.
  • Order and interpret laboratory, diagnostic, and imaging studies.
  • Develop individualized treatment plans and evidence-based care strategies.
  • Prescribe medications and therapies in accordance with state regulations and clinical guidelines.
  • Collaborate closely with physicians and other healthcare professionals to optimize patient outcomes.
  • Monitor patients between visits and ensure continuity of care.
  • Triage patient calls and provide clinical consultation as appropriate.
  • Educate patients and families on cardiac disease prevention, treatment plans, lifestyle modifications, and wellness strategies.
  • Promote heart health through community outreach, education programs, and employer wellness initiatives.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Support quality improvement initiatives and contribute to a culture of excellence, safety, and patient-centered care.

Qualifications

Required Education & Licensure

  • Graduate of an accredited School of Nursing.
  • Successful completion of an accredited Nurse Practitioner program.
  • Current DEA registration.
  • Ability to obtain and maintain professional liability insurability through MCH.

Preferred Experience

  • One or more years of experience in a clinical practice setting.
  • Cardiology, internal medicine, primary care, or related specialty experience preferred.

  • Ability to interpret diagnostic testing and formulate treatment plans.
  • Knowledge of evidence-based cardiological care practices.
  • Exceptional communication and patient education abilities.

  • Commitment to collaboration and teamwork.
  • Knowledge of applicable state, federal, and regulatory requirements.
  • Proficiency with electronic medical records and clinical documentation.
