Hospice RN - Petersburg, WV PRN

Position Details

• PRN / inconsistent position conducting home visits for hospice care
• Business hours:
  Monday‑Friday, 8:00 AM‑4:30 PM; evening/weekend hours available for on‑call
• Agency covers a 50‑mile radius; candidates must be able to see patients within this area

• Make the initial nursing evaluation in determining eligibility for hospice services during a visit within forty‑eight (48) hours of referral
• Identify the patient’s/family’s physical, psychosocial, and environmental needs and re‑assess as needed, no less than every fifteen (15) days
• Initiate and coordinate the plan of care
• Document problems, appropriate goals, interventions, and patient/family response to hospice care
• Collaborate with the patient/family, attending physician and other members of the IDG in providing patient and family care daily
• Instruct and supervise the patient/family in self‑care techniques when appropriate
• Maintain accurate and relevant clinical notes regarding the patient’s condition
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

• Current and unrestricted RN licensure in the state of practice
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle, or public transportation

• 1+ years of clinical experience
• Current CPR certification or ability to complete within 90 days of hire

Sign‑on Bonus: $5,000 for external candidates for full‑time employment.

Salary range: $71,640 to $107,460 annually based on full‑time employment. Other benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution. Eligibility requirements apply.
