Overview
Under the direction of the Youth Program Manager for Flagler Services and in close collaboration with the Rapid Re-Housing (RRH) Team, the Rapid Re-Housing Stabilization Case Manager – Youth supports young adults in transitioning from homelessness to permanent housing. This position plays a key role in developing, implementing, and maintaining the youth-focused dimension of our Rapid Re-Housing Program.
All work is performed within a collaborative, team-based model using a systems approach. Services are delivered safely, ethically, and in accordance with accrediting standards and St. Joseph’s Villa’s mission, values, and guiding principles.
This position is based at the Housing Resource Center in Petersburg, VA.
Responsibilities Intake & Housing SearchCollaborate with Flagler staff to identify homeless youth/young adults and conduct intakes in accessible community locations
Partner with the Housing Specialist to build and maintain landlord relationships willing to rent to young adults with limited or negative rental history
Participate in applicant interviews and RRH team meetings to assess housing barriers and develop initial re-housing plans
Develop individualized housing stabilization plans with clients, including realistic goals and actionable steps
Coordinate temporary housing when needed
Assess client needs related to utility deposits, furnishings, and moving expenses
Coordinate move-in logistics, including access to community donation centers and partner organizations
Conduct regular case management meetings to develop comprehensive budgets and long-term housing stabilization goals
Support clients in employment, income development, money management, and benefit access
Teach self-advocacy skills related to housing stability
Provide referrals to community resources and follow up to ensure successful engagement
Track, document, and report client progress
Identify and cultivate new community resources to support clients
Participate in weekly case coordination meetings
Educate clients on RRH program expectations and roles
Utilize assessment tools to evaluate strengths, life skills, and knowledge of tenant rights/responsibilities
Collaborate with leadership to establish measurable outcomes for youth housing services
Partner with youth-serving community agencies and Career and Transition Services (CATS) to support education and employment attainment
Prepare check requests and invoices for financial assistance (e.g., rental subsidies, utility deposits/payments)
Maintain accurate documentation in the Homeless Client Information System (HCIS)
Ensure compliance with grant-mandated documentation requirements
Attend staff meetings, case reviews, and professional development trainings
Present client updates and collaborate on solutions for high-barrier cases
Maintain positive community relationships
Perform additional duties as assigned
Required:
Bachelor’s degree in a human services field OR minimum of 4 years of relevant experience in lieu of degree
At least one (1) year of experience in:
Case management with adult clients
Working with or understanding issues impacting homeless families
Working with domestic violence survivors
Utilizing community resources and referrals
Excellent communication skills with diverse populations (youth, families, landlords, government agencies)
Ability to read and interpret contracts
Valid driver’s license with good driving record
Ability to work some evenings and weekends as needed
Proficiency in case management software and data systems
Experience working with homeless youth or young adults
Knowledge of tenant rights/responsibilities and Fair Housing laws
Experience developing household budgets
