Support Coordinator – Developmental Disabilities (DD)

Are you passionate about helping individuals with developmental disabilities live fulfilling, independent lives? We are seeking a compassionate and organized Support Coordinator to join our team. In this role, you will work directly with individuals and their families to coordinate services, develop person-centered plans, and connect them with the resources they need to thrive in their communities.

What You'll Do

  • Conduct assessments and reassessments to determine individual service needs.
  • Develop and maintain Person-Centered Plans (PCPs).
  • Coordinate medical, behavioral health, residential, vocational, and community support services.
  • Complete monthly and quarterly case management visits.
  • Monitor service delivery and ensure quality outcomes.
  • Advocate for individuals while coordinating services with families, providers, and community agencies.
  • Maintain accurate and timely documentation in compliance with state and federal regulations.
  • Assist individuals in accessing community resources, crisis services, and other needed supports.
  • Promote independence, community integration, and person-centered outcomes.

Qualifications

  • Bachelor's degree in Human Services, Social Work, Psychology, Sociology, Counseling, or a related field.
  • Meets Virginia DBHDS requirements for a Qualified Developmental Disabilities Professional (QDDP/QDIP/QMRP), or the ability to qualify.
  • One year of experience working with individuals with developmental disabilities preferred.
  • Valid Virginia driver's license with an acceptable driving record.
  • Excellent communication, organizational, and time management skills.
  • Ability to manage a caseload while maintaining strong documentation and compliance standards.
