Overview
Graham Healthcare Group is seeking a Home Health Physical Therapist for a position in Petoskey, Michigan. This role supports patients in Petoskey, East Jordan, Charlevoix and surrounding areas and is eligible for a $15,000 total bonus potential, including a $10,000 first year success bonus and a $5,000 sign-on bonus. The position is ongoing, 40 hours per week, with an 8-hour shift.

Responsibilities- Collaborate with the care team to provide management and delivery of patient care plans in a home health setting.
- Support patients in Petoskey, East Jordan, Charlevoix and surrounding areas.
- Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required.
- Licensed Physical Therapist issued by the state in which you will be working.
- Minimum of one year healthcare experience.
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
- A commitment to consistently meet critical deadlines for charting.
- The skills needed to self-manage your time and schedule.
- Demonstrated experience with tablets, mobile phones and EMR software.
