Roles & Responsibilities   Provide patient-centered evaluations and evidence-based physical therapy interventions for residents in a skilled nursing facility
Develop individualized treatment plans and measurable short- and long-term goals
Collaborate with nurses, OT, SLP and other team members to coordinate care and optimize outcomes
Document assessments, progress notes and discharge planning in a timely, compliant manner
Educate residents and families on safe mobility, fall prevention and home exercise programs
Participate in care conferences and contribute to quality improvement initiatives
Qualifications   Active Physical Therapist license in Michigan (or eligibility to obtain one prior to start)
Experience in Skilled Nursing Facility (SNF) settings preferred
Current CPR/BLS certification (or willingness to obtain)
Strong clinical reasoning with experience treating geriatric and medically complex patients
Excellent communication skills and ability to work within an interdisciplinary team
Reliable, professional, and flexible approach to scheduling and patient needs
Education   Doctor of Physical Therapy (DPT) preferred;
Master’s or Bachelor’s in PT acceptable with relevant experience
Knowledge & Skills   Skilled in evaluation and treatment of mobility, balance and gait disorders
Knowledge of SNF regulations, documentation standards and care pathways
Proficiency with therapeutic exercises, manual therapy and assistive device training
Comfortable using electronic medical records and completing timely documentation
Good time-management, prioritization and patient-education abilities
