Family Care-Service Coordinator – LTSS Service Coordinator (Case Manager)
LocationSouthwestern Wisconsin counties:
Pepin, Buffalo, Trempealeau, Jackson, Clark, La Crosse, Monroe, Juneau, Vernon, Sauk, Richland, Crawford, Grant, Iowa, Lafayette, Green.
This field‑based role enables associates to primarily operate in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. The position is designed to provide direct, hands‑on engagement and responsiveness to client needs within the Family Care Program.
Responsibilities- Conduct face‑to‑face program assessments using predefined tools to identify and evaluate clients.
- Apply motivational interviewing techniques during evaluations and coordinate an individual’s waiver (Family Care Partnership) and behavioral or physical health needs.
- Manage non‑clinical needs of members with chronic illnesses, co‑morbidities, and/or disabilities to ensure cost‑effective utilization of long‑term services and supports.
- Document short and long‑term service and support goals in collaboration with the member’s chosen care team, including caregivers, family, natural supports, service providers, and physicians.
- Serve as the single point of contact for client case management within the interdisciplinary team to provide care coordination support.
- Submit utilization/authorization requests to utilization management with supporting documentation aligned with the individual’s care plan.
- Report critical incidents to appropriate internal and external parties such as state and county agencies (Adult Protective Services, Law Enforcement).
- Assist and participate in appeals, fair hearings, member grievances, and state audits.
- Assist in training new staff and support process improvement initiatives.
- BA/BS degree and a minimum of 2 years of experience working with a social work agency, or an equivalent combination of education and experience.
- Assisted living or social work experience preferred.
- Waiver experience preferred.
- Comfortable using technology.
- Willingness to travel to work sites and other locations as necessary.
