Role Overview:
As an IRIS Consultant, you will support individuals in maintaining independence and self-direction through the IRIS program. Your responsibilities include building trusting relationships, developing IRIS Support and Services Plans, and assisting participants in understanding and maintaining their IRIS eligibility. You will conduct regular contacts and visits, connect participants with local agencies, and document all interactions promptly. The role involves working independently within a social services capacity, collaborating with a team, and potentially conducting annual functional screens.
Qualifications & Skills:A Bachelor's Degree in social work, psychology, or related fields with at least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of direct social services experience. Bilingual skills are a plus. Candidates should have a strong commitment to participant autonomy, be comfortable with technology, and serve diverse populations with empathy and respect.
Key Requirements:Valid driver's license, reliable transportation, satisfactory driving record, and ability to meet auto insurance requirements.
