Registered Nurse Inpatient Rehab Clinical Coord

St. David's Medical Center in Austin seeks a Registered Nurse Inpatient Rehab Clinical Coord to lead and deliver high-quality, patient-centered rehab care. In this full-time role, you'll coordinate clinical services for a diverse inpatient rehab population, perform comprehensive assessments, and develop individualized care plans with an interdisciplinary team. You'll serve as a clinical resource and mentor, support performance improvement and safety initiatives, and ensure accurate documentation.

Responsibilities

• Coordinate and oversee clinical care for patients in the inpatient rehabilitation unit
• Provide direct bedside nursing care, assessments, and evidence-based interventions
• Develop, implement, and update individualized care plans in collaboration with the interdisciplinary rehab team
• Serve as a clinical resource and mentor for nursing staff, supporting best practices and policy compliance
• Monitor patient progress, functional outcomes, and safety, escalating concerns as needed
• Educate patients and families on rehab goals, discharge planning, and ongoing care needs
• Ensure accurate documentation, regulatory compliance, and quality metrics monitoring
• Participate in performance improvement initiatives and staff education within the rehab unit
