Hospice RN Case Manager (2373668)
Registered Nurse Case Manager responsible for delivering the Plan of Care for each hospice patient. Full nursing responsibility includes initial evaluation, developing and coordinating care plans, documenting progress, and collaborating with patients, families, physicians, and interdisciplinary team members.
Primary Responsibilities- Makes the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
- Identifies the patient’s/family’s physical, psychosocial, and environmental needs and re‑assesses at least every 15 days.
- Initiates and coordinates the patient’s plan of care.
- Documents problems, appropriate goals, interventions, and patient/family response to hospice care.
- Collaborates daily with the patient/family, attending physician, and other IDG members to provide patient and family care.
- Instructs and supervises the patient/family in self‑care techniques when appropriate.
- Maintains accurate and relevant clinical notes regarding the patient’s condition.
- Assesses the ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition.
- Current and unrestricted RN licensure in the state of practice.
- Current driver’s license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
- 1+ years of full‑time experience as an RN.
- If not meeting RN requirement, 3 years of full‑time clinical experience as an LPN may substitute.
- 1+ years of clinical experience.
- Current CPR certification or ability to complete within 90 days of hire.
Pay ranges from $71,640 to $107,460 annually based on full‑time employment, local labor market, education, experience, and certifications. In addition to salary, benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (subject to eligibility).
