Registered Nurse - Home Health

This is a 13-week contract position.

AM shifts, Monday through Friday, with weekend rotation for on-call duties.

We are seeking a dedicated Registered Nurse (RN) with recent home health experience to join our team. The RN will be responsible for providing high-quality nursing care in a home setting.

Responsibilities:

• Conduct Start of Care, Resumption of Care, Recertifications, Discharges, and routine visits, achieving a minimum of 30 points.
• Participate in weekend on-call rotation as needed.
• May assist in other territories within the service area, which has a radius of 60 miles.

Required Experience / Certifications / Licensure:

• Active RN license with recent home health experience.
• Experience with Home Care Home Base (HCHB) required; applicants without HCHB experience may be considered at a reduced rate for the first two weeks if training is available.
