Patient Coordinator, Center for Surgical Health

The Patient Coordinator (PC) for the Center for Surgical Health (CSH) plays a central role in the delivery of surgical support services for patients by coordinating day-to-day activities of the program, supporting operational workflows and ensuring patients receive timely, compassionate, and equitable access to surgical care. CSH is a surgical support and education program developed to connect patients with access to Penn Medicine Attending Surgeons and engage students and employees in the practice of surgery.

This position will support coordination of patients in the program from referral to intake and through connection with primary care. The PC will screen patients for program eligibility, enroll patients, collect and store patient information and related documentation. PC will assist with organization of program volunteer Personal Patient Navigator (PPN) trainings, onboarding, and will proactively support PPN-patient dyads with progress during the perioperative continuum.

The day-to-day responsibilities of the PC include, and are not limited to, patient intake and enrollment (i.e., screening patients, explaining CSH services to patients and their families, confirming eligibility for program services, tracking patient progress); recruiting volunteer PPNs through marketing events and supporting volunteers with navigation steps, inputting data into CSH Salesforce; responding to operational queries in a timely manner; regularly reporting progress to managers and upon request;

organizing, maintaining, and assuring accuracy of all patient documentation.

The ideal candidate is extremely organized, detail oriented, has experience working in public health, health care, and/or social work and has an interest in higher education as the program is situated in CSH, an academic-community partnership which supports, educates and advocates for members of our community with surgical disease. As a result, it is critical for the Patient Coordinator to be passionate about the CSH mission, program activities and developing connections within the Philadelphia community to improve surgical health.

The Patient Coordinator will have the responsibilities described above working with moderate supervision.

This position is contingent upon continued funding.

· Bachelor of Science and 1 to 2 years of experience or equivalent combination of education and experience is required.
