Position: PCP Opportunity | Philadelphia | $340,000 Per Year + $75,000 Signing Bonus

The center provides Uber/Lyft to those who dont drive. Also, they offer exercise, art and other activities for their seniors to promote better health at no cost to the patient.

Role:

• Compensation:
  Base Range $265k-290k
• Annual Bonus: up to 20% of base salary
• Bonus based on: 1. Patient satisfaction rating
  2. Living the companys values
  3. HEDIS compliance
  4. Reducing hospital sick days
• Sign-on Bonus - $75K
• Primary Care 100% Outpatient
• Value-Based Care Model
• Average patients per day: 12-14
• Panel size:
  Max of 650
• Shifts:
  
  Monday-Friday 8a-5p
• Physicians have their own care team with MA/scheduler/ referral specialist and full support from leadership
