CMA​/Care Coordinator

CSI Companies is looking for Medical Assistants to assist in quality care of those with nephrology. This is a direct hire position on a Hybrid schedule. Title: Medical Assistant Pay $17/hr to $22/hr DOE Hours: Full Time on a hybrid schedule Location: Great Philadelphia, PA
Responsibilities:

• Creates, reviews, and revises patients plan of care.
• Documenting patient labs, blood pressure, blood sugar, weight, diet/nutrition, and any other health behavior than could affect outcome
• Establish patient goals and interventions
• Ensures stage appropriate referrals for education, vascular access, and transplants are completed
• Maintains consistent communication with all patients
• Participates in patient enrollment in programs by gathering all clinical elements necessary.
• Educate patients about benefits of the programs.
• Performs medication reconciliations
• Collaborates with several specialists on necessary care and services
• Acts as a patient advocate, liaison, and information resource
• Monitors and reports patient outcomes through plan of care
• Provides chronic care education for nephrology and chronic/complex conditions
• Provides culturally appropriate education
• Performs timely and accurate documentation
• Performs other related duties as assigned or needed.

Qualifications:

• Broad understanding of physician front- and back-office practices.
• Knowledge of insurance carriers, copays, and eligibility verification.
• Telephonic customer service experience preferred.
• Detail oriented, professional attitude, reliable, and strong time management skills
• Ability to interact customers in a professional manner.
• Ability to communicate effectively both verbally and in writing.
• Bilingual preferred.

Education and Experience s:

• High school diploma or equivalent.
• Medical Assistant degree, diploma, or Certificate.
• Preferred Nephology experience.
• 1 year minimum of clinical experience.
