Epicured is actively hiring Social Care Navigators to support New York’s Medicaid Section 1115 Waiver program. You will engage Medicaid members, assess eligibility for Health-Related Social Needs (HRSN) services, and coordinate referrals through Epicured and community partners.
This role reports to the Associate Director of Member Experience and is suited for organized, compassionate individuals who thrive in a high-volume virtual contact center environment.
