Summary:
The Cognitive Division & Penn Memory Center within the Department of Neurology brings together an energetic team of clinicians and researchers dedicated to the investigation and treatment of individuals with Alzheimer's disease, mild cognitive impairment and other age-related progressive memory disorders. Our center includes neurologists, advanced practice providers, genetic counselors, neuropsychologists, nurses, social workers, and clinical research coordinators. We are seeking a brain health navigation nurse that will play an integral role in patient care, education, support, and most importantly coordination of patients eligible for anti-amyloid treatment.
Modeled after the oncology nurse navigator structure, the brain health navigator will streamline the diagnostics journey for patients, coordinate logistics and support the patient in scheduling necessary diagnostic tests and navigating the health care system. The Brain Health Navigator (BHN) model emphasizes shared decision-making by providing patients with a comprehensive assessment and a range of options for care. The BHN role is designed to facilitate this process by offering personalized assessments, suggesting appropriate diagnostic testing, and connecting patients to resources and treatment options.
This nurse's goal will be to improve the treatment program efficiency and patient/care partner experience during treatment eligibility assessment, which can be a lengthy and complicated process for patients. This nurse will be working closely with providers and clinical/administrative support to reduce the time to initiate treatment and support patients and care partners when making treatment decisions. Additional and future responsibilities include assisting with the broader cognitive care journey, including the initial detection of cognitive impairment and assisting with expediting cognitive impairment early detection within primary care.
To be successful in this role the nurse would have experience in patient navigation/care coordination and/or experience with cognitive patients. It will also be important for this nurse to be efficient in navigating epic/technology. These are not requirements but experience in either of these is highly preferred. Work will be hybrid with most of care coordination happening virtually but some patient interaction likely.

- The Ambulatory Nurse II Colleague works closely with the interprofessional health care team to facilitate the coordination of care across the continuum. The nurse focuses on expanding the knowledge and skills necessary to provide individualized care based on physical, psychosocial, cultural, educational, safety, and age-appropriate considerations of assigned patients.
