Receives written/verbal shift report from RN at start of shift;
Assists in data collection for nursing history and physicals (vital signs, height/weight, blood sugar, etc.) and communicates changes to RN;
Participates in discharge planning and coordination of care with patients and families;
Maintains infection control and standard precautions; monitors patient safety and restraints as ordered;
Documents findings and interventions with signature and CNA designation on all charted entries
