Receives and relays shift reports between RN and CNA; provide patient care under RN supervision;
Assists with data collection (vital signs, height/weight, glucose, functional status) and communicates changes to RN;
Assists in planning and implementing patient care goals and communicates patient responses to the plan to the RN;
Maintains infection control and safety procedures, monitors for safety risks and applies restraints per policy as directed;
Documents findings, interventions, and patient outcomes with signature and date/time on charted entries
