RN Registered Nurse Manager for Home Health Job Summary

Deer Meadows Home Health is seeking a highly skilled and compassionate RN Registered Nurse Manager for Home Health to join our dynamic team in Philadelphia, Pennsylvania. As a key member of our home health care team, you will be responsible for providing exceptional patient care, managing a team of healthcare professionals, and ensuring the highest level of quality and efficiency in our home health services.

Nurse Manager Responsibilities

This position will be a mostly in an administrative/managerial RN capacity.

  • Manage a team of RNs and LPNs/LVNs, providing guidance, mentorship, and support to ensure quality patient care
  • Working with our clinical director and nurse educator on education initiatives
  • Light scheduling duties
  • Coordinate with healthcare providers, families, and caregivers to ensure seamless transitions and optimal patient outcomes
  • Monitor and evaluate patient progress, making adjustments to care plans as needed
  • Maintain accurate and detailed records of patient care, including assessments, treatments, and outcomes
  • Participate in interdisciplinary team meetings to discuss patient care and share best practices
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination
  • Identify and report potential patient safety issues or quality concerns to the Director of Home Health
Registered Nurse Manager Requirements
  • Currently licensed as a Registered Nurse (RN) in the state of Pennsylvania
  • Prior experience in home health care required, management experience preferred
  • Strong clinical knowledge and skills in medical-surgical nursing, with the ability to assess and manage complex patient conditions
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
  • Ability to manage a team of healthcare professionals, providing guidance and support as needed
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities
  • Proficiency in electronic medical records and other healthcare software systems
