Position: Family Nurse Practitioner - Bilingual (Spanish)

Family Nurse Practitioner - Bilingual (Spanish)

The Bilingual Family Nurse Practitioner will provide primary care services for adults and families in English and Spanish languages, according to certification, in Federally Qualified Health Center (FQHC) settings to diverse and underserved populations. The employee will report to the Medical Director.

Essential duties and responsibilities include:

  • Provide primary care services.
  • Work within all state and federal laws and regulations, and the agreement with the collaborative physician.
  • Manage acute and chronic health problems using evidence-based protocols.
  • Provide health screenings, immunizations, and other disease prevention programs.
  • Provide age-appropriate assessment and interventions to meet the needs of patients and/or families served.
  • Provides patient/family education, health counseling, and health advocacy services to patients and/or their families.
  • Be willing to work occasional weekends and on-call.
  • Collaborate with clinical staff on care and follow-up.
  • Participate, as needed, in public health management of communicable diseases in shelters and in other health initiatives.
  • Maintain positive relationships with funding sources and community organizations.
  • Perform other related tasks and duties as directed and assigned.
  • Work as a member of the Primary Care Medical Home team to provide integrated health services.
  • Document all services promptly.
  • Act in the capacity of a mandated reporter to both the State of Pennsylvania and the Joint Commission.

  • Understands and adheres to PHMC compliance standards as they appear in the PHMC Code of Conduct, Whistleblowers, and Conflict of Interest Policies.
  • Keeps abreast of all pertinent federal, state, and PHMC regulations, laws, and policies as they presently exist and as they change or are modified.
  • Comply with HIPAA and Confidentiality Policies and Procedures as they apply to the job.
  • Comply with Department of Public Health (DPH), The Joint Commission, and other accreditation and regulatory agencies' standards.
  • Adhere to all PHMC Policies and Procedures.
  • Knowledge and adherence to Infection Control and Environment of Care Guidelines and Procedures as

Job Requirements include:

  • Credential

    Required:

    Licensed as a Registered Nurse and an APN Nurse Practitioner.
  • Primary Source Verification: PA Division of Consumer Affairs, Board of Nursing.
  • Meets credentialing requirements as stipulated by Medicare, Medicaid, and private insurers.
  • Preferred credentialed with insurance companies.

Skills include:

  • Ability to read, write, and speak in Spanish at the level of proficient or above, including in medical terminology.
  • Ability to demonstrate cultural competence in Latin American cultures.
  • Ability to collect adequate client data, conduct a physical exam, interpret physical findings, and manage or refer patients as conditions or protocols dictate.
  • Good verbal communication and writing skills.
  • Computer skills.
  • Ability to communicate and work with a variety of patient populations, some with complex needs.
  • Ability to prioritize and organize workday to meet the changing needs of the program and clients.
  • Ability to work within a team framework and respond positively to team members' concerns.
  • Ability to track work to completion and compile reports.

Experience includes:

  • Three (3) years of nursing experience, at least two (2) of which shall have been as an Advanced Practice Nurse in a related specialty, preferred, but not required.

Education includes:

  • Licensed as an RN and FPN Nurse Practitioner.
  • Certified as an Advanced Practice Nurse in the State of Pennsylvania.
