Clinical Nurse
The Clinical Nurse provides direct patient care and nursing services in a dynamic, community-centered ambulatory care setting. Under general direction, the RN conducts nurse-led visits, performs clinical assessments, manages and evaluates patient treatment plans, and delivers patient education across a diverse population. This role is integral to care team function and reflects our commitment to accessible, affirming, and high-quality primary care for all patients.
Key Responsibilities- Conducts nurse-led visits and provides direct patient care, including assessment, treatment, and follow-up in alignment with clinical protocols
- Performs telephone triage, using clinical judgment to prioritize patient needs and coordinate appropriate care
- Delivers patient education and self-management support tailored to individual needs, health literacy, and cultural background
- Evaluates diagnostic results and monitors treatment plans; maintains accurate and timely documentation in the EMR
- Oversees high-risk and complex patient appointments, supporting continuity of care through timely follow-up and referral coordination
- Serves as a clinical resource and informal mentor for medical assistants and support staff
- Collaborates effectively within a multidisciplinary care team including providers, behavioral health, and social services
- Supports clinical flow and contributes to quality improvement initiatives
- Maintains active PA RN licensure and required certifications; participates in continuing education per PA State Board of Nursing requirements
- Attends departmental meetings, staff development days, and supervision as scheduled
- Graduate of an accredited RN program required; BSN preferred
- Minimum two (2) years of clinical nursing experience; ambulatory, community health, or primary care experience a plus
- Active Pennsylvania RN licensure required
- Proficiency in EMR documentation; strong computer literacy
- Strong clinical assessment skills and ability to apply sound clinical judgment independently
- Excellent oral and written communication skills; strong organizational skills and attention to detail
- Demonstrated ability to work effectively within a multidisciplinary team
- Ability to prioritize, manage competing demands, and remain flexible in a dynamic environment
- Culturally humble and committed to providing affirming, equitable care to patients across diverse racial, ethnic, socioeconomic, gender, and sexual orientation backgrounds
- Commitment to patient confidentiality in accordance with agency policy, Pennsylvania Act 148, and all applicable privacy laws
- Pennsylvania Criminal Background Clearance
- Child Abuse History Clearance
- FBI Fingerprint Clearance
- Ability to stand, sit, and walk for extended periods
- Ability to lift objects up to 35 pounds
- Effective written and verbal communication
- Visual and auditory acuity necessary for clinical practice
- Must have all required vaccinations, including annual TB screening (PPD or equivalent)
