We are seeking an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to provide primary and palliative care to home‑bound patients in the Lehigh County, PA region on a full‑time basis under physician supervision.

• Provide high‑quality care to primary and palliative patients in and around Lehigh County, PA
• Assess patients’ health through physical examinations, medical histories, and create optimal care plans
• Communicate effectively with office staff and clinical operations across internal platforms
• Deliver patient‑centered care to a panel of patients
• Develop impressions, diagnoses, and treatment plans
• Foster trust and consult with patients, families, and caregivers during visits
• Order diagnostic testing as medically necessary
• Prescribe medications, supplies, and appropriate medical interventions

• Board Certified, with a valid NP or PA certification in the state of PA
• Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose
• Positive demeanor and strong people skills
• Ability to confidently problem‑solve and operate independently
• 1–2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required)

• $110,000 – $140,000+ (depending on experience and other incentives)
