RN Nurse Navigator, AHN; Hybrid

Hybrid Role – Location

The role requires in‑person work and the candidate must live within 50 miles of the Greater Pittsburgh or Greater Erie region.

Serves as the consistent point of contact for referring physicians, patients, caregivers, families, and the community, facilitating patient flow and coordinating appointments and access to clinical and supportive care services within the Network.

• Oversee community outreach and high‑risk populations, establishing and sustaining working relationships within the network.
• Educate and coordinate care regarding patient diagnosis, treatment options, clinical trial information, and available resources.
• Work with the health care team to ensure safe handoff and coordination of care between facilities and during inpatient–outpatient transitions.
• Collaborate with High‑Risk Care Teams to reduce readmissions.
• Assist Registry staff with data collection and patient outcomes to update care delivery models.
• Coordinate appointments with multidisciplinary team members and accompany patients as needed.
• Address medication adherence issues.
• Identify at‑risk patients and facilitate appropriate screening processes in AHN physician offices/facilities.
• Trend data and outcomes for the navigation program.
• Identify gaps to improve patient care across the continuum.
• Develop or attend clinical care conferences to report active patients to the multidisciplinary team.
• Identify bereavement needs of families and develop a plan of care.
• Perform other duties as assigned or required.

• Minimum current State of PA RN licensure or current multi‑state licensure through the Nurse Licensure Compact.
• 1–3 years nursing experience in the required specialty.
• CPR – American Heart Association certification.
• Act 34 Criminal Background Clearance Certificate;
  Act 33 Child Abuse Clearance Certificate;
  Act 73 FBI Fingerprinting Criminal Background Clearance Certificate.
• Preferred:
  Bachelor’s Degree in Nursing and certification in area of expertise.
• Valid PA driver’s license may be required.
• Older Adults Protective Services Act clearance may be required depending on location.

Minimum: $34.29, Maximum: $54.25 per hour. Base pay is determined by qualifications, experience, and other factors.
