Penn Medicine - - Responsibilities:
Assist with data collection for nursing history and physicals (vital signs, height/weight, glucose) and communicate changes to the RN;
Provide patient and family information to the RN for coordination of care and discharge planning;
Assist with activities of daily living (bathing, grooming, feeding, mobility, transfer, oral hygiene);
Maintain infection control and standard precautions; monitor for safety and fall risks;
Document findings and patient outcomes in medical records with proper CNA signature and date/time
