Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Tucker House Nursing and Rehabilitation, a leading provider of long-term and post-acute rehabilitation services in Philadelphia, Pennsylvania. About the Role

As an LPN at Tucker House, you will be responsible for providing direct patient care, including administering medications, wound care, and assisting with daily living activities. You will work closely with our skilled nursing team to develop and implement individualized care plans that meet the unique needs of each resident. Our LPNs are critical members of our interdisciplinary team, and we rely on their expertise and dedication to deliver exceptional care and outcomes.

Responsibilities

  • Provide direct patient care, including administering medications, wound care, and assisting with daily living activities

  • Develop and implement individualized care plans in collaboration with the interdisciplinary team

  • Observe and report changes in residents' conditions to the Registered Nurse or other healthcare professionals

  • Participate in resident assessments, care planning, and care delivery

  • Maintain accurate and complete records of patient care and treatments

  • Collaborate with other healthcare professionals, including physicians, occupational therapists, and physical therapists, to deliver comprehensive care

Requirements

  • Current LPN licensure in the state of Pennsylvania

  • High school diploma or equivalent required

  • One year of previous experience as an LPN preferred
