The Acute Care Nurse Practitioner within the ambulatory setting works in collaboration with, and is an integral member of, a multidisciplinary health care team. The APP provides comprehensive care at the cardiac surgery outpatient practice in Center City.
Job Description
Job/Practice Description
Busy outpatient practice providing care to adult and geriatric populations- Four-day work week (Monday–Friday) totaling 40 hours
- No weekend or holiday coverage required
- Position requires Adult Gerontological Acute Care Nurse Practitioner certification
Assessment & Management
- Performs complete histories and physicals and documents in the medical record.
- Orders and collects data using appropriate assessment techniques, diagnostic information, and procedures where indicated.
- Serves as the first responder for patient status changes and situations requiring procedural or resuscitative interventions.
- Assesses risks associated with care of acute and complex chronically ill patients including medication side effects, immobility, nutrition, electrolyte imbalance, immunocompetence, and invasive interventions.
Diagnosis
- Collaborates with the interdisciplinary health care team in diagnosing acute and chronic conditions.
- Manages diagnostic tests through ordering, interpretation, performance, and supervision.
- Formulates differential diagnoses by priority and identifies complications, implementing appropriate interventions.
Plan of Care
- Identifies expected outcomes and formulates individualized, evidence-based plans of care.
- Prescribes diagnostic strategies and therapeutic interventions.
- Implements, monitors, and modifies plans of care as needed.
- May perform advanced procedures consistent with privileges and competency validation.
Documentation
- Documents all aspects of care, including histories, progress notes, procedures, and discharge summaries, in compliance with standards and timeliness requirements.
- Ensures documentation captures patient acuity and meets compliance standards.
Communication & Collaboration
- Maintains open communication and collaboration with the interdisciplinary care team.
- Communicates effectively with patients, families, and staff to ensure continuity of care across the continuum.
Professional Practice
- Demonstrates professional practice behaviors, including precepting, mentoring, and teaching.
- Engages in research and evidence-based practice initiatives.
- Participates in Advanced Practice Grand Rounds and specialty meetings.
- Maintains continuing education units and professional organization memberships.
Educational/Training Requirements
- Master of Science in Nursing (MSN) from an accredited institution
- Adult Gerontological Acute Care Nurse Practitioner certification
Certificates, Licenses, and Registrations
- Current Pennsylvania Certified Registered Nurse Practitioner (CRNP) license
- Current Prescriptive Authority in Pennsylvania
- Current DEA registration
- Current Basic Life Support (BLS) certification
