As a Certified Nursing Assistant (CNA) at Cliveden Nursing and Rehabilitation Center, you will play a vital role in delivering exceptional care to our patients. Our subacute care facility is located in Philadelphia, within walking distance of Germantown, and provides short-term post-acute rehabilitation as well as long-term skilled nursing care.

About the Role:

• Work with a dynamic and innovative team to provide personalized care to patients from several area hospitals, including Temple Hospital, Roxborough Hospital, Albert Einstein Medical, and Chestnut Hill Hospital.
• Provide direct patient care, including assisting with activities of daily living, vital signs, and mobility.
• Collaborate with our interdisciplinary team to develop and implement individualized care plans.
• Maintain accurate and timely documentation of patient care.
• Support a culture of excellence, infection control, and quality improvement initiatives.
