RN Care Manager: Adult Health & Care Coordination

Alpine Physician Partners is seeking a Care Manager (RN) to deliver clinically and psychosocially informed, person-centered care management for Medicaid members with complex needs. You will perform assessments, develop care plans, coordinate transitions of care, and navigate medical and social services with interdisciplinary teams.
